Terms and Conditions of Online Procedure Deposits
PLEASE READ: This is NOT an online appointment scheduling system. If you arrived here on your own or by accident, please exit now since this page does not apply to you. If you were asked to click through to this page by Dr. Reed or her assistant, then you are at the correct page. Read the terms and conditions below and proceed as instructed.
All procedure appointments for permanent makeup or medical tattooing are scheduled on a first-come basis. Most are scheduled during an in-person consultation with Dr. Reed at her office. They are reserved with either a credit card swiped for guarantee or a deposit paid by cash or check. Instead you have chosen to schedule your procedure differently according to one of the reasons below:
- Due to distance or other circumstances, you declined travelling to Dr. Reed’s office for an in-person consultation. However, you still want to have a procedure performed. OR
- You previously had an in-person consultation with Dr. Reed but did not schedule your procedure at that time (no deposit or credit card was taken). And now you want a procedure performed.
Step #1: You must first schedule a tentative procedure appointment date and time by telephone or email (telephone is preferred). You can do this with either Dr. Reed or her assistant. Once it has been scheduled, your appointment will be held without a deposit for only 12 hours. If you do not pay the online deposit within 12 hours of scheduling your appointment, your name will be taken off the calendar and that date and time will become available for another person.
Step #2: Read the terms and conditions below. Complete the form at the bottom of this page and click through to the secure payment website. Pay your online deposit according to the instructions. Your procedure appointment date and time will then officially be yours (unless you cancel or reschedule according to the terms and conditions below). A receipt will be emailed to you.
Online Procedure Deposit Terms and Conditions
- I agree to pay a deposit of $100.00 per procedure to officially reserve and hold the procedure date and time that has been scheduled for me.
- If I am having two procedures in the same day, I agree to pay a deposit of $200.00.
- All deposits will be applied toward (deducted from) my total procedure bill on my appointment day at checkout.
Rescheduling: I may reschedule my procedure appointment without penalty if at least 48 hours notice is provided ahead of the scheduled date and time. The deposit(s) will be held and applied toward a new procedure date. If less than 48 hours notice is provided, the deposit(s) will be forfeited (not refunded).
Cancellations: I may cancel my procedure appointment without penalty if at least 48 hours notice is provided ahead of the scheduled date and time. If I cancel with at least 48 hours notice, my deposit(s) will be refunded to my credit card. If less than 48 hours notice is provided, the deposit(s) will be forfeited (not refunded).
Late Arrivals: If I arrive late, my procedure results will be affected because lost time cannot be “made up.” If I am late for my procedure appointment 30 minutes or more, my appointment must be rescheduled. The penalty for less than 48 hours notice will apply and my deposit(s) will be forfeited.
No-Shows: If I do not appear for my appointment, I will be considered a “no-show” and my deposit(s) will be forfeited.
- I affirm that I have read and understand the above policies for procedure appointments not scheduled in person at Dr. Reed’s office.
- I affirm that I am at least 18 years of age.
- If female, I affirm that I am not pregnant or nursing now, and I will not be at the time of my appointment.
- I affirm that I do not have any medical conditions that require a clearance from my physician for cosmetic tattooing.
